To become a new patient, all you need to do is call us, 515-964-8350, we will be happy to find an appointment for you. We can file most insurances though we are in-network with Delta Dental Premier. Once you have an appointment, you will need to contact your previous dentist to get your dental record transferred and we will send you a link to our patient portal to fill out new patient forms. At your first visit, a photo ID and dental insurance card will be requested for your chart. If you have a medication list, please bring that as well. Then you can sit back and relax and let us take care of your dental needs.
